Benefits Eligibility

Solucient is proud to offer a wide array of benefits to meet employee needs. These benefits include medical, dental, vision, flexible spending accounts, short-term disability, long-term disability, life insurance, educational assistance, paid time off, an employee assistance program, a health club subsidy, pre-paid legal services, transportation benefits, and a savings incentive plan. All full-time employees are eligible to participate in company benefit plans. Part-time employees who regularly work more than 20 hours per week are eligible for most company benefits, but some are limited as described below. Generally, eligibility for benefits begins the 1st day of the month following date of hire. In addition, Solucient offers medical, dental, vision and life insurance coverage to dependents and domestic partners.

Medical Insurance

Solucient offers a PPO (Preferred Provider Organization) medical plan. In Ann Arbor, we also provide an HMO (MCare).

The PPO is administered by CoreSource and gives employees the freedom to visit any provider. It allows you to receive referral-free access to participating physicians, out-of-network coverage, and preventative care options. Using an in-network provider affords you the highest level of benefits. For in-network claims, the plan has no deductible, a $20 co-pay for physician office visits ($30 for specialists) and 90% co-insurance for most other in-network services. Out of network claims are generally paid at 70% of usual and customary costs after meeting a deductible. The PPO network is PHCS for all employees other than those living in Washington who access the Beechstreet network and those in Ann Arbor who have a choice between PHCS and Preferred Choices.

MCare-HMO (Ann Arbor employees only)

MCare is provided as an HMO option for employees that live in the Ann Arbor, Michigan area. You must coordinate your care through a Primary Care Physician (PCP) to receive benefits. Office visits with this plan are paid at 100% after a $15.00 co-pay. Prescriptions are paid on a three-tier basis: $15.00/generic, $30.00/formulary and $45.00/brand name drugs.

Prescription Drugs - Express Scripts

The PPO has prescription drug coverage through Express Scripts. There is a co-pay of $10.00 for generic drugs and $30.00 for brand name drugs, if you have the prescription filled at a participating pharmacy. You may also fill maintenance or any on-going prescriptions through the Mail Order Drug Program at a cost of $20.00 for generic and $60.00 for brand name drugs for a 3-month supply.

Dental Coverage

The dental plan offers two choices through the MetLife Preferred Dentist Program (PDP). The annual maximum for both in-network and out-of-network services is $1500 per calendar year. You can check to see if your dentist is in the PDP by logging on to www.metlife.com. Both options include orthodontia benefits for covered dependent children up to a lifetime maximum of $1500 in the PPO option and $1000 in the Co-Pay option.

MetLife Preferred Dental Program

The Dental PPO lets you visit any licensed dentist in or out of MetLife's Preferred Provider network. In-network coverage includes preventive services at 100%, basic services at 80%, and major services at 50%. Out-of-network coverage includes preventative services at 100%, basic services at 80%, and major services at 50%, but is subject to UCR (usual, customary and reasonable limits). This plan includes a $50 annual deductible on non-preventative services. Specialty care and orthodontia coverage is also provided without a referral.

MetLife Co-Pay Option

Coverage is provided through in-network and out-of-network providers (the same network accessed by the PPO). For in-network services, however, you will be charged a co-pay by your dentist. Out-of-network services will be reimbursed at 80% for preventative services, 40% for basic services and 20% for major services.

Vision Coverage

The vision plan is offered through VSP. Benefits can be accessed both on an in-network and out-of-network basis. You receive the highest level of coverage using in-network providers and services. When using in-network providers, an eye exam is paid at 100%, after a $10.00 co-pay, glasses are paid at 100% after a $25.00 co-pay (up to $120.00) and there is no co-pay for contacts (covered up to $120.00). If you decide not to see a VSP doctor, the applicable co-payment will be deducted from the amount charged before applying a reimbursement. You'll also receive a lesser benefit and typically pay more out-of-pocket. Benefits may be accessed once every 12 months.

Flexible Spending Accounts

Employees can enroll in either one or both of our Flexible Spending Accounts: Healthcare or Dependent Care Reimbursement. For the Healthcare Account employees can contribute pre-tax dollars up to $3,600 per year to obtain reimbursement for healthcare claims, deductibles, co-pays, Rx co-pays, over the counter medications, etc. - costs not covered by medical, dental, and vision plans. For the Dependent Care Accounts employees can contribute pre-tax dollars up to $5,000 per household to obtain reimbursement for dependent care costs so that you (and your spouse) can work. This includes reimbursement for daycare, summer day camp, au pairs, after-and before-school programs, etc. Employees who are enrolled in these plans will receive tax savings because they will be reimbursed with "pre-tax" dollars. Our Flexible Spending Accounts Provider is CoreSource. With CoreSource, all medical claims submitted to CoreSource for payment will automatically roll over to the flex claim department to be processed against your flexible spending account balance.

Life Insurance

At no cost to you, Solucient provides $50,000 of group term life and accidental death and disability insurance. You also have the opportunity to purchase additional life insurance for yourself and your family. Employees can purchase optional life coverage for themselves (maximum coverage is 5X your covered compensation or $500,000, whichever is less). Employees can also elect coverage for their spouse or domestic partner in increments of $5,000 up to 50% of employee's elected coverage and for their children in coverage levels of either $5,000 or $10,000 - one premium covers all children. Employee must elect voluntary life insurance coverage for themselves in order to elect voluntary life coverage for their spouse, domestic partner, or children.

Short Term Disability (STD)

STD is provided by Solucient, at no cost to you. If you are unable to work due to illness, injury or pregnancy, the plan pays 85% of eligible earnings up to a maximum of the first 90 days of disability. For absences due to injury and hospitalizations, there is no waiting period. For all other disabilities, there is a waiting period of 7 consecutive calendar days before STD benefits are paid. Disability income is taxable.

Long Term Disability (LTD)

To help you meet your financial obligations if you are unable to work due to long-term illness or injury, Solucient offers LTD coverage. LTD coverage of 50% of eligible earnings with a maximum of $4000/month is provided at no cost to all eligible employees. You can elect to increase your coverage to 67% of eligible earnings with a maximum amount of $8000/month. You must be disabled for 90 continuous days before benefits begin. LTD is only an available benefit to those who regularly work more than 32 hours per week.

Savings Incentive Plan (401K)

Solucient's Savings Incentive Plan is a 401(k) retirement plan that allows you to save for your retirement with pre-tax dollars and invest your savings in your choice of sixteen Vanguard funds. Employees are eligible to participate the 1st day of the month following 30 days of employment. Solucient matches 50% of the first 6% of eligible compensation contributed and grants matching contributions immediately. Employees vest in company contributions at 33% per year of service as defined by the Plan. Employees can choose to defer up to 50% of pre-tax income up to the maximum amount of $15,000 annually (in 2006), and can invest an additional 10% of post-tax income subject to IRS limitations.

Health Club Subsidy

To promote a healthy lifestyle, Solucient will reimburse through payroll up to $25.00/month as a subsidy for health club membership dues. You will be eligible for this subsidy the 1st day of the next month following your first day of employment. You must be an active member of a health club to receive this monthly subsidy.

Educational Assistance

Solucient encourages all employees to advance their knowledge and skills though job related educational programs. To be eligible for this benefit you must be a full-time employee working toward a degree and taking classes through an accredited educational institution or vocational technical school. The maximum annual benefit is $5,250.

Employee Assistance Program (EAP)

Solucient offers an Employee Assistance Program (EAP) through Ceridian to all employees, their spouse and immediate dependents. If you or your family has a concern that is affecting your well-being or work performance, the EAP is available 24 hours a day, 7 days a week to put you in touch with a trained professional counselor. The EAP is a completely confidential service and covers up to three face-to-face counseling visits at no cost to you. The EAP also provides financial and legal counseling and on-line tools to help you meet the demands of every day life.

The Hyatt Legal Plan

Solucient offers this pre-paid legal services plan to all employees who regularly work more than 20 hours per week. Under the plan, you and your eligible dependents receive unlimited telephone advice and office consultations on personal legal matters. Covered matters include, but are not limited to, wills and estate planning, real estate matters, uncontested adoptions and guardianships and debt collection defense.

Paid Holidays

Full-time employees are entitled to 9 paid holidays in 2006. Part-time employees receive a pro-rated benefit based on the number of hours worked. The 2006 paid holidays are:

New Years Day - January 2nd
Presidents Day - February 20th
Memorial Day - May 29th
Independence Day - July 3rd and 4th
Labor Day - September 4th
Thanksgiving (Thurs. & Fri.) - November 23rd & 24th
Christmas Day - December 25th

Transportation Benefit

The Transit Benefit program provides a tax benefit by enabling employees to purchase fares on public transportation with pre-tax earnings. Participants can pay for their commuting costs through monthly payroll deductions of up to $105. Employees can also save taxes by paying for qualified parking through a pre-tax payroll deduction.

Paid Time Off (PTO)

All employees regularly working 20 or more hours per week are eligible for personal time off. PTO accrues each pay period at the following rates according to years of service: 20 days of PTO each year during the first three years of employment; from the fourth to the 10th year, 25 days per year will accrue; and after ten years of employment, 30 days of paid time off will accrue. PTO accruals are prorated for part-time employees. The accrual rates listed below are per pay period:

0-3 years of employment 6.67 hours per pay period
4-10 years of employment 8.33 hours per pay period
10 + years of employment 10.00 hours per pay period

Solucient also offers a PTO Donation Program. This program allows you to donate your PTO time to other employees who have suffered unexpected illness, injury or tragedy in their lives.
